Registration: Eligible Professionals
Eligible professionals are required to register on the national level through the CMS website prior to completing the state application.
Register on the Medicare and Medicaid EHR Incentive Program Registration and Attestation System (R&A).
Eligible Providers will need to complete the following in order to complete registration:
- Have a National Provider Identifier (NPI) number.
- Have a National Plan and Provider Enumeration System (NPPES) web user account ID and Password
- Have an EHR Certification Number
Once Eligible Providers have the required information listed above, they can successfully register at the Medicare and Medicaid EHR Incentive Program Registration and Attestation System.
Complete the state-level application that can be accessed through your secure provider portal.

A Patient Encounter Volume template has been created to help you capture the information needed to determine volume. If you chose to use the template, please upload it as part of your application as it will assist the Agency in your pre-payment validation process.
If you will be attesting for a Physician Assistant Led Federally Qualified Health Center or Rural Health Center, please complete the Attestation for PA Led FQHC or RHC form and upload it as part of your application.
