Notifications Submitted to the Certificate of Need Program
Effective July 1, 2004, health care facilities and providers (
as defined under s. 408.032, Florida Statutes) must provide the Agency notification of:- Replacement of a health care facility when the proposed project site is located in the same
district and on the existing site or within a 1-mile radius of the replaced health care facility, if the
number and type of beds do not increase.
- The termination of a health care service, upon 30 days written notice to the agency.
- The addition or delicensure of beds not otherwise reviewed under CON pursuant to subsections 408.036 (1), (2) and (3), Florida Statutes or Chapter 651, Florida Statutes.
